Dear Healthcare Provider:

As you know, the Do Not Resuscitate (DNR) bracelet program has been in effect in Connecticut since 1991. The orange plastic DNR bracelets have been $1.00 since the start of the program. After nearly 18 years, increased costs have required us to revisit the pricing of the bracelets. Beginning September 1, 2008, the bracelets will now cost $2.00.
Enclosed, please find an updated order form reflecting the new price for the bracelets for you to share with your healthcare colleagues. If your organization is exempt from Connecticut sales tax, please remember to include a copy of your tax exempt certificate with your bracelet order.

Additionally, there is the option to order metal CT DNR bracelets. The cost for the metal bracelet is $22.00. The order forms for the plastic and metal DNR bracelets can be located on our website at www.ctacep.org.
